What can I do on the patient portal?
You can send and receive secure messages from our office staff.
You can view & pay your current balance.
You can link multiple members of your family to one portal account (no need to have an account for each person).
You can enter/update your personal information; address, phone number(s), and insurance.
Instructions for creating an account:
STEP 1: Go to www.mypatientvisit.com (or click the button above) to access our secure patient portal.
STEP 2: From the home page, click the “Create an account” box.
STEP 3: Create your Username and Password. Complete the First name, Last name, Date of Birth, and Zip fields. NOTE: The First/Last name you enter must match the name on your patient record. Our Practice ID is YVFGIB .
STEP 4: Enter Security Code provided to you by our office.
STEP 5: Read the full “Terms & Conditions” and click the acceptance checkbox. Click “Create Account”
STEP 6: Select three Security Questions from the dropdown menus, input appropriate answers, and submit. NOTE: Security questions cannot be changed once submitted.
STEP 7: Under “myInfo” click Patient Information and complete all items.
STEP 8: Under “myInfo” click Insurance Information and complete all items.
STEP 9: Under “myDocuments” click Forms; click Start Form; this is your medical history information.
STEP 10: When Steps 7-9 are completed you can Log Off
